The amount of co-payment of medical expenses has a “limit.” Therefore, if an insured person’s co-payment calculated based on a certain standard exceeds the limit, the amount exceeded will be provided as “high medical treatment expense.”

When an insured person wishes to reduce medical expenses paid at medical institution at point of contact

Documents required
Relevant persons Insured person and/or dependent under the age of 70 whose medical expenses paid at medical institutions in a month are expected to exceed the co-payment limit

Remarks Can be used for both hospitalization and outpatient services.
Those over the age of 70 will be treated in the same way by presenting elderly insurance certificate.

When annual outpatient medical expenses of those aged 70 and over increases (total annual outpatient fee)

Documents required Claim form for repayment of high-cost medical treatment (total annual outpatient fee) and co-payment certificate issuance application form
Submission due date Within two years from the day after the reference date
Relevant persons

Insured person and/or dependent aged 70 or over whose annual total co-payment amount for outpatient medical treatment (from August 1 of the previous year to July 31 of the current year) exceeds 144,000 yen

  • ※Those who fall into the income category “average” or “low income” as of the reference date (July 31) will be subject to this special exception.
  • ※A co-payment amount for the period in which the insured person and/or dependent fall into the category of “person with earned income matching that of an active employee” will not be included in this calculation.
  • ※Outpatient care after August 1, 2017 will be subject to this special exception.

Remarks Applications should be made to the insured person’s health insurance as of July 31.
If during the preceding 12 months there was any period for which the insured person and/or dependent was enrolled in another health insurance scheme, the insured person should first obtain the co-payment amount certificate from that insurer and attach it to the application form.
