An insured person and also family members depending upon an insured person may be paid insurance benefits by the health insurance scheme. These family members are called “dependents”. In order to be certified as a dependent, relevant family members need to meet certain conditions on “scope of family members” and “income.”

When to add a family member

Documents required

Note: The “Application form for a dependent living with and being maintained by an insured person” is unnecessary in only one case, when an insured person adds a newborn child as a dependent. In this case, please submit both the “Notification of (change of) health insurance dependent” and the “Transcript of certificate of residence.”

Transcript of certificate of residence (All of the members in the insured person’s household with no records omitted. Copies are accepted.)

Reasons for change and situation Attachment
When a dependent has a part-time income Copies of pay statements and bonus statements for the last three months.
If a dependent does not have pay statements for the previous three months due to having only recently started employment, etc., he/she should attach a copy of his/her employment contract and a pay statement for the last month or two.
When a dependent receives pension benefits A copy of the most recent pension amount revision notification (a copy of the annuity certificate if a dependent has recently begun to receive a pension), or a copy of the most recent pension transfer notification
When a dependent runs his/her own business Copies of the most recent Final Returns and the detailed statement of balance of payments (operations statement)
When a dependent is over the age of 16 and have no income The most recent income certificate (one with no records omitted; copies are accepted).
If a dependent lives in a municipality where only tax exemption certificates can be issued, please attach the tax exemption certificate (one with no records omitted; copies are accepted).

In addition, if a dependent has left a job within the last year, please also attach the following documents

①For those who have received unemployment allowance
Employment insurance benefit recipient qualification certificate

②For those who have ceased to receive or extended the receipt of unemployment allowance
Separation notice 1, separation notice 2 (if a dependent does not have a separation notice to hand, he/she should submit any document proving that he/she has left a job such as “Confirmation notice of employment insurance insured person qualification termination”)

When a dependent lives separately (except temporary relocation without family) A copy of proof of money transfer for the most recent three months (e.g. transfer notice, counterfoil of cash registration, details of bank passbook of account of origin for the transfer)
If a dependent has income, the transfer amount must exceed the dependent’s income.
When an insured person is about to divorce and has a dependent child Documents proving that the insured person has parental authority over his/her child (e.g. copy of receipt of divorce notification which clarifies the person in parental authority, family register (one with no records omitted; copies are accepted))

Relevant persons 【Insured person whose dependents are subject to reasons below】

  • Becomes ineligible as a dependent due to starting employment, living separately, dying, etc.
  • No longer meets certification conditions to be a dependent due to an increase in income
  • Maintenance relationship terminated due to discontinuance of money transfer

When a person becomes a health insurance scheme member in part-time employment A copy of health insurance card issued at the place of part-time employment or a copy of employment contract proving that he/she has become a member of the health insurance scheme
When the income from part-time employment increases or has increased A copy of employment contract or copies of pay statements before and after income increase
When pension income increases or has increased A copy of a pension amount revision notification (a copy of the annuity certificate if a dependent has recently begun to receive a pension), or copies of pension transfer notifications before and after pension income increase
When a person ceases to be a dependent due to starting to live separately Transcript of residence certification proving the dependent lives separately (one with no records omitted; copies are accepted)
When a person ceases to be a dependent due to find employment A copy of health insurance card issued at the place of employment or employment certification (copies are accepted)
When a person ceases to be a dependent due to getting married Documents confirming the date of marriage (e.g. copy of receipt of marriage notification, copy of family register (one with no records omitted; copies are accepted))
When a person ceases to be a dependent due to getting divorced Documents confirming the date of divorce (e.g. copy of receipt of divorce notification, copy of family register (one with no records omitted; copies are accepted))
